Sampling Order Form
For ordering sampling and lab testing - drinking water, wastewater, food, swimming pools and Legionella. Fill in the collection details and the tests required.

Collection Order Form
For ordering cleaning and disinfection of water reservoirs, piping and water mains. Fill in the system details and the scope of service needed, and we'll schedule a date to carry it out.

How to Fill In and Send
Filling in the form accurately shortens the turnaround and ensures your request is handled quickly and precisely.

Pick the sampling order form or the collection order form - based on the service you need - and download it.
Fill In the Details
Enter the customer name, company ID, site address and the tests or service required.
Sign and Save
Sign and save the form. For self-collection, keep the sample refrigerated until it is handed over.
